Case Manager

Location: Case Manager Office, Harrisonburg

Days/Hours: Monday through Friday / 40 hours a week

Supervisor: Director of Senior Services, Harrisonburg, Rockingham Region (HR)

Start: April 26, 2021

RESPONSIBILITIES: The Case Manager is responsible for providing information on services available to individuals 60 years and older, and their caregivers, that support independence and quality of life in the home for as long as possible. The Case Manager is the gatekeeper to VPAS services such as Meals on Wheels (MOW), Homemaker (HM), and Caregiver Support. Through a home visit and a comprehensive Virginia Uniform Assessment Instrument (UAI) assessment process, the Case Manager refers individuals to appropriate community services that meet the person’s assessed needs. The Case Manager is also trained and certified to assist all Medicare beneficiaries with questions, appeals, and enrollment issues through the Virginia Insurance Counseling and Assistance Program (VICAP).

QUALIFICATIONS: Successful candidates must enjoy working with older adults, have a solid knowledge of community resources relevant to older adults, have excellent computer, communication, organization, and time management skills, be able to work independently and as part of a team.  A bachelor's degree in Health and Human Services field preferred.  Relevant and applicable experience will be considered.

Programs Coordinator

RESPONSIBILITIES: The Programs Coordinator is primarily responsible for overseeing the development, planning, and execution of virtual and in person educational and social / recreational programs for the HR Region. The Programs Coordinator ensures that educational, social, and recreational activities are executed in compliance with VPAS policies and procedures.
 
QUALIFICATIONS: Successful candidates must enjoy working with older adults, have excellent computer, communication, organization, and time management skills, be able to work independently and as part of a team.  Leading exercise programs and presentation skills preferred.
