Valley Program for Aging Services (VPAS) is committed to full compliance with Title VI of the Civil Rights Act of 1964 and all related statutes. Title VI requires that no person shall, on the grounds of race, color, or national origin, be excluded from the participation in, be denied the benefits of, or be otherwise subjected to discrimination under any program or activity for which VPAS offers.
Any person who believes that he or she has been excluded from the participation in, been denied the benefits of, or been otherwise subjected to discrimination under any program or activity for which VPAS provides assistance, and believes the discrimination is based upon race, color, national origin, gender, economic status or limited English proficiency has the right to file a formal complaint.
A complaint must be submitted within 180 days of the alleged discriminatory act. You may file the complaint through email, by phone, or in writing.
It must include the following information:
• Your name, address, and how to contact you (i.e., telephone number, email address, etc.)
• How, when, where, and why you believe you were discriminated against.
• The location, names and contact information of any witnesses.
